- FSMB – Federation of State Medical Boards, gives information and application form for USMLE Step 3 examination. You can also apply for the FCVS verification, when applying for state Licensure. This is also the website to visit to request your ECFMG/USMLE exam history and Transcripts when applying to state medical boards for licensure. Transcripts cost $ 50 for two states.
